Home Health is a type of medical practice that delivers care to individuals in the comfort of their own homes. This practice focuses on providing medical treatment and therapy services to patients recovering from illness, injury, or those with chronic health conditions.
To be eligible for Home Health Care, a patient must have a physician's determination of needing skilled nursing and/or therapy services, and be homebound during the length of authorized care. This service is available to individuals of all ages.
We offer a wide range of services for both home health, and hospice patients including skilled nursing, physical therapy, occupational therapy, speech therapy, pain management, counseling, respite care, and so much more. Please see our Services page for more information about the services we offer.
Hospice care is a range of services that focus on providing comfort and quality of life for patients with a terminal illness who are nearing end of life. It emphasizes quality of life through pain and symptom management, as well as emotional and spiritual support for the patient and their family
Patients are eligible for the use of their hospice benefit upon a terminal diagnosis being provided by their physician. The patient must also agree to forgo curative treatment in favor of comfort care. See our Hospice page or more details.
