Home health care helps patients and older adults live safely at home with medical conditions like acute chronic illnesses, terminal illnesses, post-surgical events, and short-term health needs. Harbors Home Health & Hospice provides a wide range of care which can delay the need for hospitalization or for long-term nursing home care.

In general, home health care includes part-time or intermittent skilled nursing care, and other skilled care services like physical therapy, occupational therapy, and speech-language pathology (therapy) services. Services may also include medical social services or assistance from a home health aide. A doctor must order your home health care and then the home health care agency coordinates the services your doctor orders for you.
